WTF?
We just switched our insurance carrier at work.and got this e-mail from HR:
In issuing the Horizon medical cards I have noticed some errors in the spelling of names or street addresses. I am asking at this time that when you receive your packet that you please examine carefully your name and your address and let me know of any discrepancies. We will need to get these corrected so that there is no problems in processing your medical claims or any future mailings that you will receive from Horizon. Thank You
Well, I checked mine. HOLY SHIT! It said

I haven't lived there in 6 years! And I did have this HMO 6 years ago. SO I bet they put in my SS # and figuired I was back, and didn't bother to copy the info off my form. WTF?
